Description

The NHS Health Check programme aims to improve the health and wellbeing of adults aged 40-74 years, through the promotion of early awareness, assessment, and management of the major risk factors for CVD, diabetes and kidney disease and raise awareness of dementia, across the eligible population with a focus on our priority groups, which are those most at risk of developing a CVD. As such, it is aimed at reducing associated premature death, disability, and health inequalities.
