Description
The London Borough of Camden is seeking to award contracts for care and support at home services across two connected areas: Ongoing Care and Reablement. For the purposes of this procurement exercise, the Services have been divided into two Lots. Lot 1 (Ongoing Care) is further divided into five (5) sub-lots, and Lot 2 (Reablement) is further divided into three (3) sub-lots, as outlined below. Ongoing Care sub-lots: Lot 1a West Lot 1b North Lot 1c East Lot 1d Central Lot 1e South Reablement sub-lots: Lot 2a North West locality Lot 2b North East locality Lot 2c South locality The Services for this integrated model will provide: • Support to maintain or prevent deterioration in health and wellbeing which reduces the need for unnecessary hospital or residential care admission; • Ongoing Care with personal care, physical health care and support with daily living which the person cannot complete without assistance; or • Intensive support following a period of hospitalisation or treatment, which enables the person to regain confidence and improve self-care in their own home and engage with their local community.
