Description

Bournemouth, Christchurch and Poole Council (BCP Council) is seeking providers to join the Learning Disabilities, Autism, Mental Health Specialist Care and Support At Home Framework, the Framework is designed to transform the way care is delivered across our communities. This updated approach places people, their strengths, and aspirations at the heart of everything we do. BCP Council Adult Social Care's (ASC) vision is to support people to live fulfilling lives, in ways they choose, and in environments where they feel safe and connected The Specialist Care and Support Framework is committed to a progressive approach that will embed outcomes focus throughout the duration of this contract. This means moving away from the traditional 'time and task' model of care towards a model that is person-centred, flexible, and empowering.
