It provides individual assessments, regular reviews and informal social support, creating a low‑stigma, community‑based setting with easy access. Activities include supported farm placements and volunteer opportunities designed to promote wellbeing, prevent mental health crises and offer meaningful engagement. The service is open to people registered with a Bristol GP and accepts referrals from any route, including self‑referral, social prescribing, third‑sector organisations, social services and secondary mental health services.
