Description
This is a 12-month pilot contract for Carers' Together to undertake Carers' Conversation records on behalf of Middlesbrough Council. Carers' Together will be given third party access to LAS and receive appropriate training, to enable them to upload Carers' Conversation Records directly onto LAS and also undertake Support Planning and monitoring functions, in accordance with Scheme of Delegation procedures and approval.
