Description

The aim of this service is to commission housing related support within a 24 hour supported housing provision for young people aged 16-24 years. The service will enable young people to develop the skills to be able to live independently and move on from supported housing into their own home within the community. Ty Annog will provide a safe, empowering environment for young people to live to be able to gain the skills and confidence needed for the transition into independent living. A young person can reside in the scheme for a maximum of 24 months but it is anticipated they will have developed the appropriate independent living skills before this period so they are able to move on into their own accommodation within this community.
