Description
Contract to provide a responsive outreach service to rough sleepers and respond to reports of rough sleeping across the Borough of Knowsley. To provide intensive and focused housing related navigation support to rough sleepers and single homeless people. To provide floating support to 26 (may increase) bed space within shared accommodation across 11 (may increase) properties that support former homeless people with low or medium-sized support needs across the Borough of Knowsley. Interventions must support people to become tenancy ready or achieve a positive housing solution or sustain their shared housing, so that they have the confidence and skills to move on and sustain a tenancy independently. The Service Provider must ensure that the service engages and coordinates with appropriate interventions from partner agencies such as (but not limited to) Adult Social Care, Children's Social Care, Community Mental Health Teams, Drug and Alcohol Agencies, Housing Solutions Service, GP's and other Health Services.
