Description

NHS Norfolk and Suffolk Integrated Care Board (ICB) intends to procure an outcomes-focused Dementia Support Service that acts as a strategic neighbourhood partner across the dementia pathway within the Norfolk and Waveney locality. The service will contribute to improved population health outcomes, reduced health inequalities, increased community resilience and improved integration between primary care, community services, mental health, social care and the voluntary, community and social enterprise sector. The procurement will support delivery of the ICB's Ageing Well and Population Health Management priorities, enabling people living with dementia and their carers to remain independent, connected and well for as long as possible.
