Description

Cardiff and Vale University Health Board has invested in a critical programme of strategic transformation which has started to shape the future of health and care delivery across Cardiff and Vale Communities. This initiative is progressing well and is the next step in delivery, having developed and a cohesive, long-term vision and strategy that is addressing the evolving needs of the population, is responding to systemic challenges and ensuring the implementation of sustainable integrated care model. The partner will need to bring recognised expertise in community and primary care models, national and international experience, practical delivery capability, community engagement and organising expertise, strengths-based co-production, programme and project management capability, strategic leadership support and the ability to build local system maturity. The support must enable the Health Board and its partners to move from reactive, illness-based care towards a preventative, population health-driven and community-by-design model. The partner will work to programme leads to enable and support delivery of the following over the first 24 months: Programme establishment: • Rapidly establish the CbD programme with clear projects and workstreams, a defined programme plan and agreed programme management and governance structures. • Development of a shared ambition, understanding and ownership of the programme to include a robust 24-month co-production, engagement and communication plan • Development of a benefits realisation plan inc agreeing and implementing outcome measures and ensure regular reporting • Build consensus and consistency amongst all stakeholders Planning & Delivery: • Develop and agreed model of care which includes mapping and agreeing community footprints • Develop a workforce plan to enable delivery of the new model of care • Develop new approaches to clinical governance • Test the new neighbourhood model with identified first phase sites • Development of the new leadership approaches required to deliver the new model of care • Support a programme of leadership development to help ensure that the appropriate relationships, behaviours and accountabilities are in place • Introduce new ways of working amongst the front-line teams • Activate our workforce through autonomy and a quality improvement approach • Activate patients through a shift in focus • Challenge the thinking around the traditional models of delivery around outpatients, primary/community & secondary care interface, UEC models • Ground the ongoing strategy around creating health within Cardiff & Vale to effectively manage demand out of hospital • Build new approaches to the commission of a new delivery model that collectively leads to a population health risk reduction • Identify requirements for system wider enablers and work to develop solutions with partners for areas such as estates and digital • Support the development and writing of a business case to deliver the new model • Spread and scale areas of good practice provide hands on support to delivery teams where required • Support in the development of meaningful outcome metrics to form part of the CbD dashboard Bespoke Support • Assuming a critical friend role and where required providing senior level support and guidance to organisational Executives • Providing overall programme assurance • Provision of clinical experts and leaders to aid clinical conversations and adoption • Access to national policy thinking to shape the future direction of the programme • Making introductions to other partner clients to share experiences and learning
