Description
The Integrated Information, Advice and Advocacy Contract is the integrated front door service for adult social care and provides information, advice and general advocacy around social care, health, wellbeing and social welfare. The service provides early help to residents who often have multiple needs ranging from financial difficulties, health needs and social care support. Acting as the front door for social care, the service provides an effective screening and support function to manage demand into social care. In providing good information and advice, directing people to community services for support and maximising opportunities for residents to help themselves, the service is delivering good outcomes for residents and is significantly reducing the need for more costly social care intervention as well as reducing waiting times for social care intervention. The contract will be a contract for 3+1 years through a competitive tender
