Description
Homecare, Rapid Response and Reablement services to enable and support individuals mainatin levels of independence within their home environment within the City of London. Home Care, Rapid Response and Reablement services are delivered with the aim of supporting and enabling vulnerable adults to maximise their levels of independence at home and within the commnunity. These services can help to lessen the escalation of needs, assist in the prevention of hospital admissions and avoidance and support the individual following discharge from hospital. Homecare services in particluar can potentially reduce admission to high cost residential care settings for the Corporation’s residents. This suite of services also supports the NHS hospital discharge (D2A) model, reducing hospital length of stay, improving outcomes for people (often better recovery within the home environment) and reduce acute bed capacity. These services are delivered in accordance with a Care Act assessment and subsequent care and support plan, as well as a financial assessment for clients above income thresholds paying or contributing towards their respective care costs.
