- At the take-off end of a blown film extruder machine a hydraulic arm actuated to unload a full reel of product onto a skip on the floor. At the machine the plastic film was not adequately cut and was gathering in greater quantity, so the operator stepped over the hydraulic arm to attempt to sort out the problem. The hydraulic arm did not discharge the full reel of product on the pallet. It returned to the machine, trapping the employee between the full reel of product and the rotating drive drum. He was unaware of the emergency stop cord positioned above his head. He sustained a crushed kidney and lacerated liver which bled into the abdomen.
