- You loaded a stacked container at height, used a forklift truck that could not lay down the small access platform within the container, used a damaged pallet to carry the small access platform, did not provide measures to prevent workers falling from the open edge of the container, and did not provide supervision which implemented planned control measures, and in consequence whereof an employee stepped from the container onto the pallet loaded with the small access platform causing the pallet to topple, and employee to jump to the ground to his severe injury.
