- Maintenance technician, whilst attempting to deal with a breakdown on a large hydraulic power press, came into contact with the unguarded, rotating flywheel. As a result sustained serious (and proving to be fatal) head injuries. Joint Police and HSE WRD investigation learnt that for a number of years the guard that would have prevented the incident was never replaced and senior management were knowingly permitting employees to work within close proximity of the unprotected flywheel.
