- The deceased person was struck by a road sweeper driven by an employee as it reversed and he died as a result of catastrophic crush injuries. The company failed to take reasonably practicable measures to control the risk of their employees being struck by moving plant. There were uncontrolled vehicle movements on site, no implementation of exclusion zones, inadequate training and supervision, and inadequate site security. There was no effective system in place for controlling vehicle movements whilst on site.
